29 CFR 1926.100(a): Employees working in areas where there is a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns, shall be protected by protective helmets.  An employee was exposed to struck by hazards while handing materials up to another employee located on an above platform and the employer failed to ensure that protective helmets were utilized.   There is no abatement certification or documentation for this item.

29 CFR 1926.451(g)(1): Each employee on a scaffold more than 10 feet (3.1 m) above a lower level shall be protected from falling to that lower level. Paragraphs (g)(1)(i) through (vii) of this section establish the types of fall protection to be provided to the employees on each type of scaffold.   An employee performing residential carpentry operations was exposed to a 10.6 foot fall while working from a carpenter bracket scaffold and the employer failed to ensure that fall protection was utilized.   Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).

29 CFR 1926.1053(b)(13): The top or top step of a stepladder shall not be used as a step.  An employee performing residential carpentry operations was exposed to a fall between 6 feet and 10.6 feet while utilizing the top step of a bi-fold ladder to access a work platform.  The employer failed to ensure that the ladder was propery used.  Abatement certification is required for this item in accordance with the requirements of 29 CFR 1903.19(c).
