29 CFR 1926.20(b)(2): The employer did not initiate and maintain a safety program which provides for frequent and regular inspections of jobsites, materials, and equipment to be made by a competent person (i.e., a person who is capable of identifying existing and predictable hazards in the surroundings or working conditions which are unsanitary, hazardous, or dangerous to employees, and who has the authority to take prompt corrective measures to eliminate them): (a)Office Tech LLC at 4440 E Colfax Avenue, Denver, CO: On and preceding 7/7/15, Office Tech LLC did not initiate and maintain a safety program which provides for frequent and regular inspections of jobsites, materials, and equipment to be made by a competent person. Office Tech LLC workers removed a commercial sign from the perimeter of a roof prior to preparation for painting of the structure. The workers were exposed to fall hazards. Office Tech LLC did not perform inspections of the worksite and equipment. This condition exposed the workers to 9.8 and 13.3 foot fall hazards.

Office Tech LLC workers removed a commercial sign from the perimeter of a roof prior to preparation for painting of the structure. One worker was positioned upon the roof of the building while removing the sign. The worker was not protected by a guardrail system, safety net system, or personal fall arrest system. Office Tech LLC did not ensure that the worker utilized fall protection. This condition exposed the worker to a 13.3 foot fall hazard.
