This violation occurred on or about July 6, 2015, specific written lockout/tagout procedures were not developed for the control of potentially hazardous energy when performing servicing and/or maintenance operations on equipment such as, but not limited to, the Bullard Vertical turret lathe #635, Matsuura horizontal machining center #326 and Matsuura horizontal machining center #327 exposing employees to caught in and struck by hazards.

This violation occurred on or about July 6, 2015, the employer did not conduct a periodic inspection of the specific lockout/tagout procedures for each machine that requires lockout/tagout during servicing or maintenance activities exposing employees to the hazards of unexpected energization of equipment.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks. This violation was observed on or about July 6, 2015 on the following machines: 1) The spindle on the Bullard Dynatrol 56 inch vertical turret lathe was not adequately guarded exposing employees to being caught in the machinery. 2) The chuck on the Mori Seiki lathe was not adequately guarded exposing employees to being caught in the machinery.
