Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system:  a) On the roof of the "fellowship hall," on or about at times prior to July 10, 2015, the employer did not provide for fall protection, in the form of any of the means as listed in standard, for employees who were installing shingles on a 4 pitch sloped roof that measured fifteen from the ground level to the peak and eleven feet from the eve to the ground level, exposing employees to the hazards associated with broken bones, lacerations, sprains, and bruises.
