29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems:  a. At the construction site during shingling, 401 Taylor Ave., Park Hills, MO, on 7-9-15, fall protection was not used (note: the building was not residential and the roof slope was measured over 4:12 with a potential fall of 17 ft. 5 in.).

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  a. At the construction site during shingling, 401 Taylor Ave., Park Hills, MO, on 7-9-15, a ladder from the ground to the building section adjoining the office to the church, did not extend the required 3 ft., but was less than 1 ft.
