29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury:   a) 233 La Grande Avenue, Fanwood, NJ 07023.  Employee was operating a pneumatic nail gun while installing new framing when he was not wearing any eye and or face protection exposing him to eye and or face injuries from flying debris.    Condition occurred on or about 07/09/15

29 CFR 1926.416(a)(1): Employees were permitted to work in proximity to electric power circuits and were not protected against electric shock by de-energizing and grounding the circuits or effectively guarding the circuits by insulation or other means:    a) 233 La Grande Avenue, Fanwood, NJ 07023. Employee was working directly under an energized 13 KV overhead power line while not being protected against electrical shock by de-energizing and grounding the circuit of effectively guarding the circuits by other means.    Condition occurred on or about 07/09/15.
