29 CFR 1926.1053(b)(15): Ladders were not inspected by a competent person for visible defects on a periodic basis and after an occurrence that could affect their safe use:  a)  At the jobsite, on or about 7/8/15:  An employee was using a 6 foot step ladder where the manufacturers instructions had become unreadable, and therefore impossible to follow by the employee.  b)  At the jobsite, on or about 7/8/15:  A 6 foot step ladder was available for use where the manufacturers instructions had become unreadable, and therefore impossible to follow by an employee.    Abatement certification must be submitted for this item.

29 CFR 1926.1060(a): The employer did not provide a training program for each employee using ladders and stairways, as necessary, which would enable each employee to recognize hazards related to ladders and stairways and train each employee in the procedures to be followed to minimize these hazards:  a)  At the jobsite, on or about 7/8/15:  An employee was standing on the top level of a 6 foot step ladder where the manufacturers instructions were unreadable, and where that employee had not been trained to recognize the hazards related to safe use of ladders.  Abatement certification must be submitted for this item.
