29 CFR 1926.651(k)(1): An inspection of the excavations, the adjacent areas, and protective systems was not conducted by the competent person prior to the start of work and as needed throughout the shift:      Super Excavators project site located at 980 Waube Ln. Green Bay, WI: On  July 13, 2015, the competent person did not conduct inspections of the excavation and adjacent areas throughout the shift as conditions changed, such as the changes in soil conditions, safe egress, and ensuring that the sloping method was correct for the soil condition to protect employees from cave in during sewer install was not addressed.

29 CFR 1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with 29 CFR 1926.652(b) or 1926.652(c):       Super Excavators jobsite located at 980 Waube Ln. Green Bay, WI: On July 13, 2015 employees were installing a new concrete storm sewer manhole in an unprotected 6 feet deep and greater excavation exposing employees to side wall collapse. The employer did not meet the sloping requirement for type C over type B soil that was declassified by water infiltration and equipment vibration to class C soil type and the location where employee were working was sloped steeper than 45 degree.
