29 CFR 1910.23(c)(1): Open-sided floors and/or platforms four feet or more above adjacent floor or ground level were not guarded with standard railings (or equivalent) and toeboards: Note:  Toeboards are only required when persons can pass, there is moving equipment or there is equipment with which falling materials could create a hazard.    a) Air Compressor Storage Area:  On or about July 9, 2015, employees access an above ground work platform to service an air compressor, approximately nine feet above a lower level, but are not protected from falling by any means of fall protection.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:    a) The Pass Door Work Area: On or about July 9, 2015, an employees were using a Grizzly Vertical Band saw which had the non-working part of its blade unguarded, creating the potential for a severe laceration in the event of blade contact.
