29 CFR 1926.404(f)(6): Grounding path. The path to ground from circuits, equipment, and enclosures were not permanent and continuous: a) 2500-D Warwick Blvd. Newport News, VA, 23607 at Virginia International Terminals, CrossGlobe Transport, Ltd. Warehouse "B" Loading Dock: On or about 07-14-15 and times prior thereto, a 50 foot temporary extension cord with cord connectors was in use and missing the grounding pin contact. The cord set was plugged into a 6 inch right angle grinder. The cord had other visible damage with the outer jacket compromised exposing a black, white, and green ground wire contained within the temporary extension cord. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employee(s) and their representatives were informed of this abatement.

29 CFR 1926.1053(b)(4): Ladder was not used only for the purpose for which it was designed: a) 2500-D Warwick Blvd. Newport News, VA, 23607 at Virginia International Terminals, CrossGlobe Transport, Ltd. Warehouse "B" Loading Dock: On or about 07-14-15 and times prior thereto, a six foot fiberglass "A" Frame style ladder was in use in the closed position and leaning up against the loading dock. Personnel were descending the ladder while it was in the closed position to get off of the loading dock area to the ground below. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employee(s) and their representatives were informed of this abatement.

All 120-volt, single-phase 15- and 20-ampere receptacle outlets on construction sites, which are not a part of the permanent wiring of the building or structure and which are in use by employees, did not have approved ground-fault circuit interrupters for personnel protection: a) 2500-D Warwick Blvd. Newport News, VA, 23607 at Virginia International Terminals, CrossGlobe Transport, Ltd. Warehouse "B" Loading Dock: On or about 07-14-15 and times prior thereto, a 50 foot temporary extension cord with cord connectors powering a 6 inch right angled grinder was in use and not protected by a GFCI Circuit. Cord was plugged directly into warehouse shore power. The cord had other visible damage with the outer jacket compromised exposing a black, white, and green ground wire contained within the temporary extension cord as well as a scraped area with an unprotected bare conductor wire visible. The employer did not have an assured equipment grounding conductor program. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employee(s) and their representatives were informed of this abatement.
