29 CFR 1926.501(b)(14): Each employee working on, at, above, or near wall openings (including those with chutes attached), where the outside bottom edge of the wall opening was 6 feet (1.8 m) or more above lower levels and the inside bottom edge of the wall opening is less than 39 inches (1.0 M) above the walking/working surface, is not protected from falling by the use of a guardrail system, a safety net system, or a personal fall arrest system:  a) At 500 Liberty Landing Circle in Farmington, MO 63640 - On or about 07/14/2015, employees were exposed to elevated fall hazards while working near wall openings, where the outside bottom edge of the wall opening was approximately 20 feet above a lower earth surface and the inside bottom edge of the wall opening was approximately 22 inches or less above the walking/working surface.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:  a) At 500 Liberty Landing Circle in Farmington, MO 63640 - On or about 07/14/2015, the employer failed to provide a training program for each employee exposed to fall hazards of approximately 20 feet to a lower earth surface while working near wall openings on the third floor of a newly constructed apartment building, to enable each employee to recognize the hazards of falling and the procedures to be followed to minimize these hazards.
