S, Mosinee, WI 54455, the employer had not developed and implemented a safety and health program that provided for frequent and regular inspections of the job site, materials, and equipment.  The employer also did not develop and implement a safety and health program to ensure that employees recognized and the proper procedures were followed for fall hazards greater than 6 feet, tripping hazards for holes that measured less than 6 feet in depth, and the proper use of ladders on construction sites.

S, Mosinee, WI 54455, the employer did not ensure that each employee engaged in roofing work had been trained in the fall protection requirements to identify and implement conventional or personal fall protection while working on floor holes from approximately 6 feet to 7 feet and greater from the foundation.
