29 CFR 1910.178(a)(4): Modifications and additions which affect capacity and safe operation of powered industrial truck were performed by the employer without the manufacturer's prior written approval: a.) Sentinel Management, Inc. dba Semco Carbon located at 3000 Leavitt Road Lorain, OH; On or about July 14, 2015, the employer did not ensure the stabilizers used for lifting round pieces of granite were approved by the forklift manufacturer. Employees are required to use the stabilizers to pick up pieces of carbon and place them on the flipper or table, and are exposed to hazards associated with overloading a forklift and loss of stability.
