29 CFR 1926.451(b)(2)(i): Each ladder jack scaffold work platform was not at least 12 inches wide:  a) 933 N 5th Street, Philadelphia, PA - The employee was working from a ladder jack scaffold platform that was 9.5 inches wide. The ladder jack scaffold was approximately 12 feet long and 25 feet above the ground. This hazard exposed the employees to a fall hazard of approximately 25 feet. This condition was observed on or about May 5, 2015.

29 CFR 1926.451(e)(1): Where scaffold platforms were more than 2 feet above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers or stairway-type ladders was not used for access:  a) 933 N 5th Street, Philadelphia, PA - The employer did not provide safe access for the employees to reach the working level of the scaffold. The employees were working from a ladder jack scaffold platform, approximately 25 feet above the point of access. This hazard exposed the employees to a fall hazard. This condition was observed on or about May 5, 2015.
