29 CFR 1910.67(c)(2)(v): A body belt was not worn and a lanyard attached to the boom or basket when working from an aerial lift:  a) Hotel parking lot, Allentown, PA - On or about July 7 2015, two employees were working from a Genie S-60X extensible boom aerial lift, approximately 20 feet above grade, without either of the employees being tethered to the basket by a lanyard attached to a body belt or Personal Fall Arrest Harness, exposing the employees to serious injury in the event of ejection from the aerial lift's basket.
