29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems: On or about June 16, 2015,at the house under construction located at lot 118 10751 Song Bird Drive, Baton Rouge, Louisiana, the employer allowed employee(s) to conduct roofing activities on a 7 in 12 pitch roof without fall protection. The measured height from the eve of the roof to the ground was approximately twelve feet.

29 CFR 1926.1053(b)(1): Where (a) portable ladder(s) was/were used for access to an upper level landing surface and the ladder's length did not allow the ladder side rails to extend at least 3 feet (0.9 m) above the upper landing surface being accessed, the ladder was not secured at its top to a rigid support that will not deflect, and/or a grasping device was not provided to assist employees in mounting and dismounting the ladder. On or about June 16, 2015, at the house under construction at lot 188 10751 Song Bird Drive, Baton Rouge, Louisiana, the employer allowed employee(s) to access the roof of the residence without the side rails of the ladder to extend three feet above the landing while performing roofing activities. The approximate height from the eve of the roof the the ground was twelve feet.
