29 CFR 1926.451(g)(1): Employees on scaffolds more than 10 feet (3.1 m) above a lower level were not protected from falling to that lower level by fall protection established in paragraphs (g)(1)(i)-(vii) of this section:  a) 743 New Dover Road, Edison, NJ: Employees were allowed to perform stucco application operations  approximately 12' and 18' from the ground below on a fabricated frame scaffold without fall protection.  Condition occurred on July 20, 2015.

1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, structure, personnel hoist, or similar surface was not used.  Crossbraces were used as a means of access:  A) 743 New Dover Road, Edison, NJ: Employees were exposed to fall hazards of approximately 12" and 18" to the ground below when a ladder was not provided for safe access to a fabricated frame scaffold.  Condition observed on or about July 20, 2015.

29 CFR 1926.451(b)(1): Each platform on all working levels of scaffolds was not fully planked or decked between the front uprights and the guardrail supports:   a) 743 New Dover Road, Edison, NJ:  Employees were allowed to perform stucco operations from a fabricated frame scaffold approximately 12' and 18' from ground below that was not fully planked from the front upright to the rear upright.    Condition observed on or about July 20, 2015.  PL STONE & STUCCO, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.451(b)(1), which was contained in OSHA inspection number 313982910, citation number 1, item number 3 and was affirmed as a final order on July 25, 2011, with respect to a workplace located at 2377 Rt. 22W, Scotch Plains, NJ.
