29 CFR 1910.23(e)(1):   Standard railing(s) did not consist of top rail, intermediate rail and/or posts and/or did not have a vertical height of 42 inches (106.7 cm) nominal from upper surface of top rail to floor, platform runway, or ramp level:   a)  Establishment - On the Operator's Platform of Baler #2:  The employee at the operator's posiition was exposed to "over-the-railing" falls of 5 feet, 2 inches into the baler's engine components as the railing at the operator's platform was only 32 inches high.                      Violation observed on or about 06/26/2015.

29 CFR 1910.212(a)(1):   Machine guarding was not provided to protect operator(s) and other employees from hazard(s) created by:   a)  Establishment - On the tipping floor in the area of the pit conveyor for Baler #2:  Employees were exposed to injuries from scrap materials moving along the pit conveyor where body parts of workers could be caught in-between scrap and the stationary wing walls of the conveyor or between stationary or stuck scrap and other moving materials.                             Violation observed on or about 06/26/2015.
