29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs, with unprotected sides and edges 6 feet (1.8 m) or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems:  On or about 21 July 2015, and at times prior thereto, five roofers were installing asphalt shingles on the roof of the apartment complex without the use of fall protection, exposing the workers to a fall hazard of approximately 20 feet to the ground below.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  On or about 21 July 2015, and at times prior thereto, five roofers were using a 24 foot extension ladder to gain access to the roof top to install shingles the ladder was not extended three feet or three rungs above the upper landing, exposing employee to a fall hazard of approximately 20 feet.

29 CFR 1926.1053(b)(16): Portable ladders with structural defects, such as, but not limited to, broken or missing rungs, cleats, or steps, broken or split rails, corroded components, or other faulty or defective components, were not either immediately marked in a manner that readily identifies them as defective, or tagged with "Do Not Use" or similar language, and were not withdrawn from service until repaired.  On or about 21 July 2015, five roofers were using a 24 foot extension ladder to gain access to the roof top to install shingles and the ladder was used with a broken/cut rung.
