Along the exterior walls, planking on the tubular welded scaffolding extended approximately 3 inches past its supported point, exposing employees to 5 foot fall hazard, on or about 7/21/2015.

Along the exterior walls, cross and or diagonal bracing was not used on tubular welded scaffolding, exposing employees to 5 foot fall hazard, on or about 7/21/2015.
