29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.   On or about July 22, 2015, during the construction of a road bridge at a job-site located at Riverside Drive, I-90 Bridge, Lakewood, Ohio, the employer did not ensure an employee was protected from fall hazards varying in heights of approximately 6 to 19 feet, by utilizing a fall protection system/device while providing construction observation services.
