29 CFR 1910.178(l)(1)(ii): The employer did not ensure that each operator had successfully completed the training required by paragraph (l), except as permitted by paragraph (l)(5), prior to permitting an employee to operate a powered industrial truck.    Throughout the store, BigJoe motorized pallet jack: on or about July 22, 2015, operator's training in truck-related and workplace-related topics was not provided to ensure competency in safe operation of the equipment. Employees where exposed to struck-by hazards where a motorized hand truck was used for handling products.    Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).

29 CFR 1910.303(g)(1)(ii): The required working space about electric equipment rated 600 volts, nominal, or less to ground, was used for storage:  Southeast, stock room - on or about July 22, 2012, access to electrical panels was blocked by product and other materials.   Abatement certification is required of this item in accordance with the requirements of 29 CFR 1903.19(c).
