29 CFR 1926.28(a): Appropriate personal protective equipment was not worn by employee(s) in all operations where there was exposure to hazardous conditions.  At this commercial construction site, employees were not wearing the appropriate personal protective equipment for the work performed.  Employees were exposed to skin irritation and inhalation hazards while mixing  the Lehigh white cement.

29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, sturcure, personnel hoist, or similar surface was not used.  Crossbraces were used as a means of access.  At this commercial construction building site, employees did not have a ladder or other safe means to gain access to the scaffold platforms.  Employees were exposed to slips and falls of approximately 7 feet or more.

Employees were exposed to serious inhalation, skin and cancer hazards.
