29 CFR 1910.178(l)(4)(ii)(B): The employer shall provide refresher training to any operator that has been involved in an accident or near-miss incident:    The employer is failing to provide refresher training to operators who were involved in a forklift accident resulting in the injury of an employee.  This was most occurrence of this took place at the facility located at 9320 J Street, Omaha, NE 68127, where the employer has an employee operating a forklift without having been provided refresher training after an accident involving the operator and an employee of another company.
