29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net system, or personal fall arrest system:    a) 100 Cass St., Springfield, MA: On or about July 22, 2015 several employees, while working to build the 3rd story of this multi family residence, were  exposed to falls ranging from 9.9 feet to 20.7 feet to the lower level or ground below (respectively), without any fall protection.

29 CFR 1926.1052(c)(1)(i): Stairways having four or more risers or rising more than 30 inches (76 cm), whichever is less, were not equipped with a handrail system along each unprotected side or edge:    a) 100 Cass St., Springfield, MA: On or about July 22, 2015 neither of the staircases, which were the only paths to the work location on the upper floors at this residential construction site, each which had in excess of 10 risers, were equipped with a handrail system of any kind.
