29 CFR 1926.100(a): Employees working in areas where there is a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:    (a)Carillo Trimmer, Jesus at 3393 Princeton Place, Broomfield, CO 80023:  On and before July 18th, 2015, the employer allowed an employee working on the ground to work under an employee on a pump jack scaffold without the use of a hard hat.  The condition exposed employee to the potential for head injuries cause by struck-by hazards.

The condition resulted in fall hazards of approximately 20 feet.
