29 CFR 1926.651(c)(2): A stairway, ladder, ramp or other safe means of egress was not located in trench excavations that were 4 feet or more in depth so as to require no more than 25 feet of lateral travel for employees: a) 725 Washington St, Philadelphia, PA - The employee working in a trench approximately 7 feet deep, 5.5 feet long, by 2.5 feet wide did not have a means of egress from the trench, thus exposing the employees to cave-in hazards. This condition was observed on or about July 23, 2015.

29 CFR 1926.651(k)(2): Where the competent person found evidence of a situation that could result in a possible cave-in, exposed employees were not removed from the hazardous area until the necessary precautions have been taken to ensure their safety: a) 725 Washington St, Philadelphia, PA - The employees were working in an trench approximately 7 feet deep, 5.5 feet long, by 2.5 feet wide in which the competent person did not make a determination of a possible cave-in hazards and safe egress from the excavation. This hazard exposed the employees to cave-in/struck by hazards. This condition was observed on or about July 23, 2015.
