The portable ladder was not secured at its top to a rigid support, and a grasping device, such as a grabrail, was not provided to assist employees in mounting and dismounting the ladder:  a) 664 Amboy Avenue, Woodbridge, NJ 07095. Employees were exposed to fall hazards while using an portable aluminum extension ladder to access the roof when the ladder was not extended three feet above the landing and was not secured.   Condition occurred on or about 07/09/15.
