29 CFR 1910.95(c)(1): The employer did not administer a continuing, effective hearing conservation program as described in 29 CFR 1910.95(c) through (o) whenever employee noise exposures equal or exceed an 8-hour time-weighted average sound level of 85 decibels measured on the A scale, or equivalently a dose of fifty percent:  The employer did not administer a continuing, effective hearing conservation program, including, but not limited to, noise monitoring, an audiometric testing program, employee training and employee access to a copy of the standard, for employees that were exposed at or above an 8-hour time-weighted average sound level of 85 decibels measured on the A scale.  a) A thermoforming machine operator was exposed to an 8-hour time-weighted average sound level of 87.5 decibels measured on the A scale.  b) A thermoforming machine operator was exposed to an 8-hour time-weighted average sound level of 87.8 decibels measured on the A scale.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:   a. Press operators on the #5 machine were exposed to a crushing hazard created by the opening platen and the barrier guard when removing the molded part from below the extruder.   b. Press operators on the #1 machine were exposed to a crushing hazard created by the opening platen and the barrier guard when removing the molded part from below the extruder.
