29 CFR 1910.134(m)(1): Records of medical evaluations required by 29 CFR 1910.134 were not retained and made available in accordance with 1910.1020:  a) On or about July 24, 2015, Diamond Coring Company, Inc. did not retain medical evaluation results for employees that are required to wear half-mask, negative-pressure respirators while performing cutting and sealing of concrete joints.   In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).

29 CFR 1910.134(m)(2)(ii): Fit test records were not retained for respirator users until the next fit test was administered:(  a) On or about July 24, 2015, Diamond Coring Company, Inc. did not retain the record of the qualitative or quantitative fit tests administered to employees that are required to wear half-mask, negative-pressure respirators while performing cutting and sealing of concrete joints.   In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).
