29 CFR 1926.1052(c)(1)(ii):  Stairways having four or more risers or rising more than 30 inches (76 cm), whichever is less, were not equipped with a stairrail system along each unprotected side or edge:    (a) Center of the Building - on or about July 24, 2015 - the stair system from the first story floor to the second story floor was not equipped with handrails.  The elevation from the first to the second story floors was approximately 10 feet.

29 CFR 1926.1060(a)(1)(iii):  The employer did not ensure that each employee using ladders and/or stairways had been trained by a competent person in the proper construction, use, placement, and care in handling the ladders and/or stairways:    (a) Center of the Building - on or about July 24, 2015 - the employer did not provide a training program to employees that involves fall prevention on stairways.
