Detroit Ave. Toledo, OH: The employer did not have a plan or program to identify job site hazards which assists employees in maintaining a safe working environment. Such programs would support the employer and employees in identify and avoiding potential hazards such as, but not limited to falls during roofing operations, decreasing the probability of employee injury and/or illness.

29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system. Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system: a) John Henning on or about July 24, 2015, at the address of 4704 N. Detroit Ave. Toledo, OH: The employer failed to ensure employees were protected from falls while performing roofing activities atop a low sloped commercial structure. Employees removing old roofing material from atop the structure were exposed to a 9 ft. fall to the ground below.
