29 CFR 1910.147(c)(4)(i): Procedures were not utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section:  a) On or about 07/20/2015 on Line 4; where an employee clearing a clogged powder dispensing machine was injured when the air cylinder controlling the horizontal gate valve "cycled", cutting off the tip of the employee's finger. The employer did not assure that the equipment was deenergized and locked in the "off" position before the unclogging took place.  NO ABATEMENT CERTIFICATION REQUIRED
