29 CFR 1910.217(d)(4): Guide post(s) on mechanical power press(es) which were separated from their bushing by more than one fourth inch were not protected by point of operation guards or devices in accordance with 29 CFR 1910.217(c);   a) On or about 07/27/15 in the power press area; employer failed to protect employee operating mechanical power press F-00 ( full revolution - foot pedal activated ) and reaching into the die area to insert and remove parts, from an amputation hazard when guide posts on the die were separated approximately 3/4 of an inch, created a caught between hazard and no guarding method was used.      b) On or about 07/27/15 in the press area; employer failed to protect employee operating mechanical power press F-3 ( full revolution press ) operated by two palm buttons and reaching into the die area to insert and remove parts, from an amputation hazard when the guide posts on the die were separated approximately an inch, created a caught between hazard and no guarding method used.        c) On or about 07/27/15 in the press area; employer failed to protect employees operating mechanical power press C-2 ( full revolution mechanical power press ) operated by a two palm buttons and reaching into the die to insert and remove parts, from an amputation hazard when the guide post on the die were separated approximately 1.5 inches created a caught between hazards and no guarding method was used.
