Worksite located at 16901 E. 24 Highway, Independence, Missouri- Extension cord was run through an aluminum doorway in which the door was smashing the cord which could breach the insulation and energize the doorway.

29 CFR 1926.451(c)(2)(iv): Front-end loaders and similar pieces of equipment were used to support scaffold platforms when they were not specifically designed by the manufacturer for such use:    (a). Worksite located at 16901 E. 24 Highway, Independence, Missouri- Employees were exposed to fall hazards in that the forklift was not specifically designed to lift personnel. The platform that was elevated was not designed in accordance with any manufacturer's specifications or in accordance ASME specifications.
