The work rests was not kept adjusted closely to the wheel with a maximum opening of one-eighth inch to prevent the work from being jammed between the wheel and the rest, which may cause wheel breakage.     a)  Prestige Maintenance, Joint Base Langley AFB, Dourand Loop Compound and Equipment Shelter, Langley AFB, VA:  On or about 07-29-2015 and times prior thereto, a Delta ShopMaster brand Class A, Type 1 6 inch diameter wheel bench mounted grinder/sharpener, Model # GR275, Serial # 035283 was located inside a Connex box being utilized as a tool room/storage room/work shop.  The work rest on the grinding wheel was non-existent therefore not adjusted within one-eighth inch of the grinding wheel.  Grinder was used as needed weekly to eliminate burrs and grind off bolts.    Abatement certification required within 10 days after abatement date.  The certification shall include a statement that abatement is complete, date and method of abatement, and states employee(s) and their representatives were informed of this abatement.

29 CFR 1910.305(a)(2)(x): Flexible cords and cables shall be protected from accidental damage, as might be caused, for example, by sharp corners, projections, and doorways or other pinch points:    a)  Instance #1, Prestige Maintenance, Joint Base Langley AFB, Dourand Loop Compound and Equipment Shelter, Langley AFB, VA:  On or about 07-29-2015 and times prior thereto, a 50 foot temporary extension cord was plugged into an energized 110v receptacle inside the contractor trailer and passed through a partially open window across sharp metal framing and traveled downward and across a gravel parking area to supply power to a refrigerator kept outside under the equipment shelter.  This cord had a landscaping equipment trailer tire parked on top of it while on the gravel parking surface and had been kinked.    b)  Instance #2, Prestige Maintenance, Joint Base Langley AFB, Dourand Loop Compound and Equipment Shelter, Langley AFB, VA:  On or about 07-29-2015 and times prior thereto, a 50 foot temporary extension cord was plugged into a 110v receptacle at the contractor trailer and traveled across the gravel parking surface to a Connex Box to supply power to tools.  The cord-set entered the Connex box through the partially opened front door and was unprotected from the tight sealing metal door jam and closure.  Cord was kinked and had been "pressed" into the gravel parking area by equipment being driven on top if it.    Abatement certification required within 10 days after abatement date.  The certification shall include a statement that abatement is complete, date and method of abatement, and states employee(s) and their representatives were informed of this abatement.

Temporary cord was plugged into a 110 receptacle inside and passed through window screen hole in a partially opened window with sharp metal tracks.  Temporary cord was not protected from sharp edges of metal window frame and tracks.  b)  Instance #2: Prestige Maintenance, Joint Base Langley AFB, Dourand Loop Compound and Equipment Shelter, Langley AFB, VA:  On or about 07-29-2015 and times prior thereto, was using a temporary extension cord to supply power to an adjacent Connex box.  A temporary wire extension cord was plugged in the construction trailer and ran across the gravel parking area and entered a metal Connex style box through the partially opened metal doors and frame.  Abatement certification required within 10 days after abatement date.  The certification shall include a statement that abatement is complete, date and method of abatement, and states employee(s) and their representatives were informed of this abatement.
