29 CFR 1910.134(c)(1): In any workplace where respirators are necessary to protect the health of the employee or whenever respirators are required by the employer, the employer did not establish and implement a written respiratory protection program with worksite-specific procedures. The program must be updated as necessary to reflect those changes in workplace conditions that affect respirator use:  Facility: When employees were issued half-mask elastomeric respirators as protection against the effects of silica-containing dust, the employer did not also establish and implement a written respiratory protection program as outlined in 29 CFR 1910.134(c)(1)(i) through (ix).

29 CFR 1910.134(h)(2)(i): All respirators shall be stored to protect them from damage, contamination, dust, sunlight, extreme temperatures, excessive moisture, and damaging chemicals, and they shall be packed or stored to prevent deformation of the facepiece and exhalation valve.  Worksite: The half-mask elastomeric respirator used by the Foreman was stored with contamination inside the mask.  The unit was stored inside an open-topped plastic bag in the rear of the company vehicle.  This allowed silica-contaminated dust and dirt to accumulate on the mask and inside the facepiece.

29 CFR 1926.453(b)(2)(v): A body belt was not worn and a lanyard attached to the boom or basket when working from an aerial lift:  Worksite: When an employee was conducting masonry repointing activities on the side of a building, a harness and lanyard was not connected to prevent the worker from falling out of the basket.
