29 CFR 1926.651(k)(1): Daily inspections of excavations, the adjacent areas, and protective systems were not made by a competent person for evidence of a situation that could have resulted in possible cave-ins, indications of failure of protective systems: a) Jobsite - The employer did not perform inspections of the excavation or any part of the excavation with a competent person exposing the employees to cave-in hazards. Noted on 22 JUNE 2015.

29 CFR 1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with 29 CFR 1926.652(c). The employer had not complied with the provisions of 29 CFR 1926.652(b)(1)(i) in that they (a) Jobsite - Employees used a structural ramp located in between the excavation walls and steel concrete forms without shoring or other means of cave-in protection. Excavation site measured fifty (50) feet wide by fifty (50) feet long and ten 10 feet deep. Observed on or about 22 June 2015.

Ramp constructed of loose spoil at the pitch of approximately 29 degrees to 38.30 degrees. Observed on or about 22 June 2015.
