29 CFR 1910.179(n)(1): Crane(s) were loaded beyond the rated load (maximum load for which the crane or individual hoist was designed and built by the manufacturer and shown on the equipment nameplate(s)  a)  On 06/15/15, in the service department, while unloading a rear load body, using two A-frames with hoist attachments, one of the hoisting devices collapsed under the weight, injuring an employee in the process.  The hoisting capacity was set at 14,000 lbs. and the load was 18,000lbs.  ABATEMENT CERTIFICATION REQUIRED

29 CFR 1910.305(j)(1)(i): Fixtures, lampholders, lamps, rosettes, and receptacles located within 8 feet of the floor had live parts exposed to employee contact:    (a)   On or about 06/25/15, in the second floor conference room, a floor receptacle did not have a faceplate or cover to guard against accidental contact with live parts (wiring and terminals).  NO ABATEMENT CERTIFICATION REQUIRED
