29 CFR 1926.20(b)(4): The employer shall permit only those employees qualified by training or experience to operate equipment and machinery.  On or about August 3, 2015, an employee was operating a Genie S-65 aerial lift while another employee was in the lift without being trained by the employer, exposing the employees to the hazards associated with operating the lift. Hazards include, but are not limited to falls and exceeding the capacity of the machine resulting in tipping of the machine.

29 CFR 1926.416(e)(1): Worn or frayed electric cords or cables shall not be used.  On or about August 3, 2015 an employee was utilizing a 120-V Dewalt impact drill to attach ornamental steel over-hangs on the exterior of the building under construction where the outer sheathing on the cord was damaged and frayed to the insulated wires, exposing the employee to an electrical shock hazard.
