29 CFR 1926.1053(b)(1): Because of the portable ladder's length, the ladder side rails did not extend to at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access:      Mark Wills dba Mark Wills Construction had an employee exposed to 11.5 ft. fall hazards while exiting and accessing the roof via an extension ladder that  only extended 1.5 ft. above the roof surface, on a residential construction site in Mahomet, Illinois.
