29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents:   On or about August 5, 2015, at a worksite located at 6973 Pearl Road, Middleburg Heights, OH during construction activities including but not limited to roofing activities on a steep roof, the employer did not ensure employees were protected from eye hazards by ensuring eye protection was available and worn during nailing, cutting and weatherproof material installation activities.

29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems:   On or about August 5, 2015, at a worksite located at 6973 Pearl Road, Middleburg Heights, OH during commercial roofing activities, the employer did not ensure employees were protected from fall hazards approximately 8 feet, by ensuring a guardrail system, safety net system, or personal fall arrest system was being used.
