29 CFR 1926.404(b)(1)(i):  Employer did not use either ground fault circuit interrupters as specified in paragraph (b)(1)(ii) of this section, or an assured equipment grounding conductor program as specified in paragraph (b)(1)(iii) of this section to protect employees on construction sites:  (a) Front of the Building, On or about August 5, 2015 - electric tools were not protected by a Ground Fault Circuit Interrupter (GFCI).  Tools include; DeWalt DW368 Circular Saw and a Hitachi C121SH Chop Saw.

29 CFR 1926.451(g)(1)(i):  Each employee on a boatswains' chair, catenary scaffold, float scaffold, needle beam scaffold, or ladder jack scaffold was not protected by a personal fall arrest system:  (a) Front of the Building, On or about August 5, 2015 - no fall protection was available for the employee that was performing vinyl siding installations, working from a ladder jack scaffold system at a 13 foot, 8 inch elevation from the ground.
