On or about August 4, 2015, employees in the saw room were exposed to the point of operation while operating the Acra CNC Mill with Anilam 1400 Control. e. On or about August 17, 2015, employees were exposed to the point of operation while operating an Eagle Mill (9793) with Anilam 1400 Control in the CNC room. f. On or about August 17, 2015, employees were exposed to the point of operation while operating Eagle Mill (9791) with Anilam 1100 Control in the CNC room. g. On or about August 17, 2015, employees in the CNC room were exposed to the point of operation while operating a Cintimatic Crusader Series (41449) mill. h. On or about August 17, 2015, employees in the CNC room were exposed to the point of operation while operating a Bridgeport Mill with Analam Wizard 211 control. i. On or about August 17, 2015, employees in the CNC room were exposed to the point of operation while operating a mill in the CNC room, whose make was not readily identifiable but was operated by an Anilam Series 1100 Control. j. On or about August 4, 2014, employees in the saw room were exposed to the unused portion of the blade on a Wellsaw Model 850 horizontal bandsaw that was missing guarding behind the blade and had unadjusted guarding in the front of the blade. k. On or about August 4, 2014, employees in the horizontal room were exposed to the unused portion of the blade on a DoAll C-10 (344-77331) horizontal bandsaw.
