29 CFR 1910.23(a)(8): Every floor hole into which persons can accidentally walk shall be guarded by either:  (i)  A standard railing with standard toeboard on all exposed sides, or  (ii)  A floor hole cover of standard strength and construction. While the cover is not in place, the floor hole shall be constantly attended by someone or shall be protected by a removable standard railing.    a) At the detailing area, on or about 8/6/2015: A floor hole was not covered, exposing employees to trip and fall hazards.    b) Throughout the service department, on or about 8/6/2015: floor holes were not covered, exposing employees to trip fall hazards.

29 CFR 1910.157(c)(1):   The employer shall provide portable fire extinguishers and shall mount, locate and identify them so that they are readily accessible to employees without subjecting the employees to possible injury.        a) At the service department, on or about 8/6/2015: A fire extinguisher was not readily accessible as it was blocked by equipment.

29 CFR 1910.157(g)(2): The educational program to familiarize employees with the general principles of fire extinguisher use and the hazards involved with incipient stage fire fighting was not provided to all employees upon initial employment, and at least annually thereafter:        a) At the Lowery Bros., Chrysler-Jeep service shop located at 647 West Genesee Street, Syracuse, NY 13204, on or about 8/6/2015: The employer did not train its employee annually on fire extinguisher use and incipient stage fire hazards.    Abatement Note:  By this date the employer must either correct the alleged violation(s) or implement a Fire Safety Policy, as outlined in 29 CFR 1910.38 and 29 CFR 1910.39 which includes the evacuation requirements of 29 CFR 1910.157(b).     Lowery Bros., was previously cited for a violation of this occupational safety and health standard, which was contained in OSHA inspection number 314350794, citation number 1, item number 3, and was affirmed as a final order on 12/2/2011, with respect to a Lowery Bros., Infinity service shop located at 736 West Genesee Street, Syracuse, NY 13204
