OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees.    Location - 25 Carlton Street, Salem, MA 01970.    On or about 8/6/2015, an employee was exposed to serious crushing, amputation, asphyxiation hazards or even death when working on an unguarded Power Take-Off (PTO) of an operating dump truck. The employee had two fingers amputated when he went underneath the dump truck in an attempt to repair a damaged PTO; when his glove was caught in the rotating shaft of the PTO causing the amputations.
