29 CFR 1910.26(c)(2)(vii): Ladders having defects were not marked and taken out of service until repaired.  37 Shattuck Road, Hadley, MA: On or about August 6, 2015 employees were observed using ladders which were defective and had not been marked and taken out of service exposing employees to Fall Hazards.  Specific abatement documentation that this violation has been corrected must be provided within 10 days of the abatement in accordance with 29 CFR 1903.19(d)(1).

29 CFR 1910.132(a): Protective equipment was not used when necessary whenever hazards capable of causing injury and impairment were encountered  37 Shattuck Road, Hadley, MA. On or about August 6,2015 employees were working on a residential roof over 11 feet above the next lower level without being provided protective gear or barriers to prevent employees from being exposed to fall hazards.   Specific abatement documentation that this violation has been corrected must be provided within 10 days of the abatement in accordance with 29 CFR 1903.19(d)(1).
