Forever 21 Retail, Inc. was previously cited for a repeat violation of this occupational safety and health standard, 29CFR 1910.22(a)(1), which was contained in OSHA inspection number 990215, citation number 001, item number 001 and was affirmed as a final order on 1/5/2015, with respect to a workplace located at 1540 Broadway New York, NY 10036.      Forever 21 Retail, Inc. was previously cited for a violation of this occupational safety and health standard, 29CFR 1910.22(a)(1), which was contained in OSHA inspection number 922344, citation number 001, item number 001 and was affirmed as a final order on 4/10/2015, with respect to a workplace located at 1540 Broadway New York, NY 10036.

29 CFR 1910.37(a)(3): Exit route(s) were not kept free and unobstructed:  a. Stock Room Exterior Door- Employees were not provided with access to an emergency exit when boxes of clothing, bags of trash, stacks of empty cardboard boxes, and other miscellaneous items were stored in front of the rear emergency exit rendering it inaccessible.     b. Stock Room to Sales Floor Door- Employees were not provided with adequate access to an exit when boxes of clothing were stored in the doorway which separated the stock room and the sales floor, leaving an opening of only 20 inches.   Forever 21 Retail, Inc. was previously cited for a repeat violation of this occupational safety and health standard, 29CFR 1910.37(a)(3), which was contained in OSHA inspection number 922344, citation number 002, item number 001 and was affirmed as a final order on 4/10/2015, with respect to a workplace located at 1540 Broadway New York, NY 10036.  Forever 21 Retail, Inc. was previously cited for a repeat violation of this occupational safety and health standard, 29CFR 1910.37(a)(3), which was contained in OSHA inspection number 924027, citation number 001, item number 001 and was affirmed as a final order on 2/27/2014, with respect to a workplace located at 1 Garden State Plaza Paramus, NJ 07652. Stock Room - Employees did not have sufficient access to electrical equipment in the event of an emergency requiring the termination of power when boxes of clothing, bags of trash, and stacks of cardboard were stored directly in front of an electric panel and the main power disconnect.
