29 CFR 1910.23(a)(2): Ladderway floor opening(s) or platform(s) were not guarded by a standard railing with standard toeboard(s) on all exposed sides (except at the entrance to opening) with passage through the railing either provided with a swinging gate or so offset that a person could walk directly into the opening:    (a)  Ladderway by engine room:  On or about August 3, 2015 and times prior thereto, the ladderway did not have a swing gate or offset to prevent an employee from walking into.  This exposed employees to a 12 foot fall to the surface below.    (b)  Ladderway by breakroom:  On or about August 3, 2015 and times prior thereto, the ladderway did not have a swing gate offset to prevent an employee from walking into.  This exposed employees to a 12 foot fall to the surface below.    (c)  Ladderway by engine room:  On or about August 3, 2015 and times prior thereto, the ladderway did not have a swing gate offset to prevent an employee from walking into.  This exposed employees to a 12 foot fall to the surface below

29 CFR 1910.119(d)(3)(ii): The employer did not document that equipment complies with recognized and generally accepted good engineering practices.    (a)  Engine room:  On or about August 3, 2015 and times prior thereto, the doorways and wall penetrations were not sealed to prevent ammonia from leaking out if there was an inadvertent release of ammonia in the engine room.  This exposed employees to inhalation hazard of ammonia if the were in the area when the release occurred.

29 CFR 1910.119(d)(3)(i)(B): The employer's piping and instrument diagrams were not accurate and did not represent equipment that was existing and was part of the process.  Engine room:  On or about August 3, 2015 and times prior thereto, changes to the flow diagram had not been updated to reflect a change to oil cooler relief system that had been completed by Stellar Corporation.
