29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on flat roofs with unprotected sides and edges 6 feet or more above lower levels was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, warning line system, or a safety monitoring system.    115 Walnut Street, Philadelphia, PA 19103 - Employees performing roofing operations on the roof of a 4 story residential structure were exposed to fall hazards of approximately 40 feet without the use of a fall protection system, on or about 6/17/2015.     Jay Bee Roofing was previously cited for a violation of this occupational safety and health standard or its equivalent standard CFR 1926.501(b)(10), which was contained in OSHA inspection number 565918, citation number 1, item number 1, group a, and was affirmed as a final order on 2/16/2013, with respect to a workplace located at 1944 Lombard Street, Philadelphia PA.
